The manager can keep shirking his duties because YOU keep doing it for him. Next time you see a problem, go to the Manager and say "Get over there and put a stop to this". Don't allow him to keep sloughing off his job duties. If he doesn't have the guts to manage people then maybe he needs to be reassigned to a job without any supervision duties.
Have you confronted the manager below you and addressed these behaviors? I would make sure you mention some sort of consequence that will happen if they continue to disregard the rules. It's not your job to pick up on everyone's slack. We need leaders like you who keep everything together, but you shouldn't be doing other people's jobs!
Do you have the authority to fire this manager? This manager should be backing you up. That would be unacceptable for him/her to have this attitude. I would first meet with them and make this clear to them.
