You should seek a medical professional and start there.
I'm very lucky to have a 100% remote job for just this reason. I suffered two strokes from a covid complication, which would be problematic in-office but is very easily managed at home. I don't have much advice for you, I just wanted to let you know that I understand the feeling entirely and I bet you could find small changes to make a big difference (like asking for requests to be made over email rather than in-person, maybe?).

The request over email is a good idea, considering it's a very sensitive subject and I'm emotionally unstable, I will cry lol
I get confused easily and she has to repeat herself a few times. I've never answered Yes on the EEO forms when applying for jobs because we all know how AI can sometimes filter those apps out. I just don't know how to prove to her that I am actually smart, because I'm starting to feel dumb.

I'm assuming you are extremely new to HR, given your lack of knowledge about the ADA and the interactive process.
Tell your manager, NOW (and hope it's not too late), that you have a disability that affects your memory or whatever the effects of your brain injury are, and that you need an accommodation to help you perform the essential functions of your job. That's step 1.
Step 2 is to meet with your neurologist and let them know you are seeking accommodations and work and ask them to help you formulate a list of possible accommodations.
Step 3 is to give that list to your manager.
Step 4 is to have the conversation that is known as the interactive process.
