I feel you OP. Am trying to stick it out in my current location for at least another year before I'm skilled enough to get a better job elsewhere. Good luck to you!

Definitely check out SD the pay is good and the agencies are almost all "anti-agencies".. The city is nice, start looking!
Not a lot of account roles in LA unless you go sales. Switch to media buying or strategy and there is definitely a need.
Do you have automotive experience? Look into Saatchi LA or Dallas. I know they need AE

Start with your agency’s network and reach out to and becomes with people that live in that city. Then plan a visit. Make it both fun and a chance to network. Being in the city in person helps a lot
At your level, chances are no one is going to relocate you. So your current location while applying elsewhere may be turn off enough. If you have the means or place to stay (friend’s couch even) make the move and then start applying while living there. Or make it clear that you have are (concrete plan) moving to x city in y month while applying
