Use ladybugs!
If it's indoors, try a systemic insecticide like imidacloprid (e.g. Bonide insect control granules). The plant absorbs it through the roots and it kills insects feeding on plant tissues (like mealybugs). Obviously not safe for food plants, but not an issue for pothos. Best not to use it outdoors, since it can be a serious hazard to pollinating insects if it's absorbed by a flowering plant (including weeds).
If you're not comfortable using a conventional insecticide, UC ANR recommends spraying with 10-25% isopropyl alcohol weekly (test first on a small area in case it hurts the plant!), which can apparently get through the waxy coating on the insects. They note that oil- and soap-based sprays may be less effective on older insects due to a thicker wax layer, so that may be part of your issue. Sprays are tough either way because insects like to hide in inaccessible locations. This may be most useful in combination with a systemic.

You refrigerate the lady bugs, spray them with a solution to stick their wings, and release a few of them onto your plant in the evening hours. They’ll wake up and eat all your pests.
The Big 4 approach would be to deploy 2,000 ladybugs without any forethought - don’t do that 😉.
I used dish soap once and it helped, think I should have done the treatment a few times though. No more mealy bugs but plants are still sad

Qtip dipped in alcohol to get each individual off. Wipe all leaves well then bonide then regular neem spray. I’ve been able to save my rope hoya and keep the mealies from spreading.

Did the alcohol and sprayed with a mixture and still have them. They hide so well.
I’m going through this with my Hoya krimson queen..she’s 7’ long. I’m using a 70% rubbing alcohol spray all over (keep out of light until it dries or it will burn) and also using a qtip with rubbing alcohol and dab the mealy bugs I can see. I do this every 2-3 days. I’m on my 3rd week.
