I would ask her previous teacher for some samples of her work from the end of the year. The goal is to get her to 6th grade level, with maybe “refresher exercises” from last years curriculum. What does her current IEP state? What are the goals that were listed? Resource is such a hard job.
Do you have assessments such as the Brigance to use to assess her current level? I would look at her IEP goals/objectives and progress reports from last year to see what she was working on last year and how she was progressing towards meeting her goals.
There should be records (IEP) from previous years that will give you a place to start. I’d give benchmarks assessments to get a baseline ( fluency probes like an IRI, Reading Naturally, a math fluency probe, I’ve used placements from Number Worlds, even the curriculum your district uses should have some placement type assessments). You use the CC to give you grade level standards and scaffold it to match your students’ needs. Our ela curriculum has a program that provides the same passages but it’s scaffolded to be 1-2 grade levels below. The curriculum also has information about meeting the different tier levels

I start by looking at data from the previous years if it available. Easycbm has a lot of practice things you can use and begin pin pointing where the student is at fluency and comprehension wise. Readworks.org has a lot of grade level readings you can do with her once you figure out where she’s at. My school uses IXL for math and language arts, they have a diagnostics test on there that she can do and it’ll recommend lessons for intervention as well.

As a teacher, I am sure you have been doing a great job. By having the wisdom to decipher where she’s at and do your best to take her there is a plus. Get in touch with old teachers, assess, set a plan for the end of the school year, stick with it until you meet your goal.
We use FastBridge for reading and math! I love it!! We screen 3x per year and then use the same program to progress monitor!
At the start of the year I have a list of scaffolded skills in ela and math. I typically look at their IEP and test them a step or two below where the IEP says they are since they lose so much in the summer. It also helps build confidence when they get to the harder stuff.
We take their IEP goals and assess at the end of the year, and then again at the start of the year. It's hard to work on goals without first determining their current level of performance. They may have a grade level goal, but that does not mean that you have to use grade level materials. Use the materials that align with their present level. For example, we have 3rd graders that we use 1st grade curriculum with. I hope this helps. :)
Look at last IEP. The goals should give you some ideas where she is. Give her some assessments for math and or reading like iReady or make some fifth grade level informal tests.
