What do folks with family (SO, kids) opt for in terms of Google health insurance: a high-premium / low(er) deductible PPO plan, or a low-premium / high(er) deductible HSA plan?

Kid requires 5-8 annual pediatric visits, while SO and I frequent 5-10 visits each between PCP (annual physical) and specialists (dermatology, chiropractor, etc.)

Coming from a low-premium, low-deductible MBB healthcare plan, so realize that will be taking a hit either way. Just trying to minimize the damage.

Why would you ever want to rollover your 401K into an IRA? It seems like doing so would essentially prevent you from successfully being able to do a IRA to Backdoor Roth IRA conversion due to the pro-rata rule. Am I missing something? Wouldn’t you always want to just rollover your old 401Ks to your new 401K?
