I know most salary posts here are geared towards big law but 1850 billable requirement with base salary of $85,000. NJ based 20 attorney firm. Thoughts?

I am a PI attorney who has far too many clients to manage. This is due to having 4 major surgeries over the last 2 years and my sudden loss of a very experienced paralegal and inability to find a similar replacement. I stopped taking new cases a while ago and still feel like I need to dump some of my more time consuming cases but I worry about doing so, for a myriad of reasons. At the same time, I feel like a failure and am sleeping less and less at night. How do I get out of this nightmare?
