In my opinion— yes. You can always shoot for the stars! But if you’re not hearing back — Look for some mid-sized agencies and get a few years under your belt and then try again.

It's not impossible to land a job with a bigger agency right out of the gate, but it's definitely harder. I'd go with something smaller so you can gain some experience and build up your portfolio.
Where are you based? My NYC agency is a consumer agency but is just getting into health. Could see a world where you transition in house or come in and tell them your desire to do more consumer work?

Know the right people, keep in touch with people who have left your agency for other ones, work on the right accounts that get traction. when one of those agencies gets something pharma or health related (or if they’re doing something like pro bono work for a vaccine push or wins a hospital group) use that as a jumping off point
it’s harder to get in as a director than as a midlevel, because there’ll be an expectation that you’ve won awards etc. in my experience
