They needed a faster intervention, but the rules slowed things down. I had to choose between sticking to the book or advocating for what I felt was right. It’s left me questioning how we balance policies with patient care. For those who’ve faced similar ethical dilemmas, how do you navigate them? Do you push back, or do you follow protocol, even when it feels wrong for the patient?

I’ve been there, and it’s such a tough spot to be in. I try to follow protocol as much as possible because policies are in place for safety, but when it comes to patient care, I’ll always advocate for what I think is best.

It helps to gather evidence to support why a different approach might be better, then present it to the team or supervisor. It’s important to communicate effectively without being confrontational. Policies are important, but our role is to advocate for the patient, and sometimes that means challenging the system.

Honestly, I think hospital policies can sometimes do more harm than good. In situations like that, I’ll push back if I know there’s a better option for the patient, even if it means going up the chain of command.

Is it too much to ask a home health company for someone to shadow/orient? I was just hired PRN and that was something I asked for during the hiring process and the recruiter agreed. 6’0 and weighed in at 220lbs today. All time high and have gained 20lbs since gyms closed in March. When I did workout it was more or less 100% compound lifts. I’m now limited to a few kettlebells but am not even close to as consistent as I was. Any idea on how I can lose this excess 20lbs? I feel like it crept up over night.

I'm a PA in the PNW working for a private group staffing ED's in several hospitals. Recently the group has asked the PA's to take on a couple new roles (triaging and rapid track from triage i.e. straightforward ESI 4, 5). With these changes the PA's are considering asking for increased pay in the form of an addition of a COLA (cost of living adjustment) to our normal salary.
