I have unlimited pto and we don't have to ask, we put in the request and add it to our company wide pto calendar simultaneously. It's approved by management right away with no issues. Unless there is some important training that is mandatory they will pause pto during that time period unless the request was made well in advance, otherwise it's granted with no questions asked.

We do have to essentially submit a request, however it is more of a formality to get it officially approved. Unless there is some odd or rare circumstance it really doesn’t matter much. I can see some positions though that need coverage, that would be a different story completely. Every company seems to approach it differently though.
We also have to put in and management does ask that there's no more than half the team off at a time because someone does have to cover us when we're out. Which makes a lot of sense. But very rarely. Is our PTO denied?.
I ask my team to let me know before they put in a request. I almost never say no, but I like to look at the big picture before telling them to go ahead and submit it.
What is the company policy /procedure for time off? Merck is a corporation, so I would think that they have a specific way of doing so. Inasmuch, I think the company wide process and schedule would be visually available for all in a department.
I am considerate of my team and work load but PTO is my right as an employee so I will inform my manager of my time off way in advance but I’m not asking for it.
We don’t have to ask. We check our company calendar and if no one else is scheduled off we can request it. It gets approved immediately then we just notify our counterparts and that’s that.

The way it’s set up allows us to take PTO and make changes to our schedules without having to bother our manager. But basically yes our manager still has to approve our request so we can get paid our PTO.

We put in a request and basically it’s a first come first basis, if I want a specific day but someone else already got approved for that same date then no I don’t get it off, basically supervisor let us know if it was approved or not. We used to have a calendar where everyone would put in their requests just so the rest of us would see what days were already taken but not everyone cared to write down the days so eventually we stopped doing it.

A lot of the comments are shocking to me. How can you work in healthcare and not rely on your coverage agreeing to be available?
Sounds a bit of micromanging. Unless there's an issue with scheduling and coverage I don't think it needs to run by manager before putting in request. PTO is a company benefit and unless there is a company wide policy that states must ask management before putting in request then I don't see why asking is needed. If I decide I need tomorrow off and I don't have an upcoming dealine or mandatory meetings then I just put in my request and take off. My PTO is earned and given by employer so I take as needed. Now if I'm taking a full week of vacation I may mention to manager that I'm taking off this week & put in my request but I normally submit request 1-2 weeks in advance when taking more than 1 or 2 days.

OK so basically you request and dont assume, but it is generally approved.
Yes, we request it off and it gets approved by a 3rd party that our company uses to track our PTO and create our schedules. Our manager doesn’t know we have submitted the request until we notify via calendar invite. Our manager is the one who approves the PTO request for time card purposes (after it has already been approved by 3rd party) but really she doesn’t care so long as there are not multiple people out at one time. The only time she will get involved is if we have an emergency and need to be off on a day that someone else is off. But we usually work that out amongst ourselves first and if one of us can’t move our PTO then she comes in and can override the request and allow us to both be off (or not).
Set a policy around leave requests. Only approve them if the request follows policy. Present the policy kindly and for the purpose of team clarity.
Required to put in two weeks in advance a request it has never been denied. But if I didn’t do what was required I wouldn’t except it.
Every position I’ve had, we simply put in a request and it’s either approved or not. I’ve never had pto requests denied. I think it depends on the company and how requests are expected to be submitted, but with most companies implementing online platforms these days, there’s no sense in asking your boss to utilize the platform that’s in place for requests to be made.
I send my supervisor an email with what dates I plan on taking off, not so much for approval as we have PTO & sick time that can be utilized. Then I send out a calendar invite so my whole department is aware that I will be off.
Theres usually 3 of us on a shift and many a time, someone calls off leaving the other 2 to hold down the fort. Lord forbid I want to submit pto though, because suddenly our boss wants us to do a swap instead because she feels the two remaining on the shift are not a strong combo. She has no basis for this aside from secondhand prejudice from gossip and rants of the dayshift. So everyone else can submit normal pto except for me because of alleged competency issues of the two remaining on shift if I call off.
For me it's simple common sense. 1st, I ask my boss (via email) for upcoming PTO, If I'm sick, I let him know (via email or text) I'll be off due to being sick, prior to my shift. I've accumulated lots of vac/sick time, so it's no issue.
But for me it's simply professional and common courtesy to take these simple steps. My my boss, coworkers deserve to know if I'll be off ahead of time. To date, no one has complained.
