Most companies that have pay grades have a minimum and a maximum. Come review time people below the (assuming average performance) minimum get more than average; over but below mid point average; above midpoint lower to none. The theory being once you hit midpoint you should be getting promoted to the next grade.
As a canidate, I presume the midpoint as the max I can really expect (adjusted for COL differences). They can go higher, but it some times takes an act of God with multiple levels of approvals.
This is 100% on target. As a hiring manager, I aim for something close to the middle of the range for most new hires. Folks with exceptional credentials (ie highly relevant experience, highly relevant higher education, etc) may come in a bit higher than the mid point
I always assume 10-15 % lower as norm. What I have heard is there are some practitioners who extremely fit into the next role but lack in one or 2 aspects.
Say in consulting you may come in as industry expert with strategy consulting experience but may miss components of digital transformation. Due to your lack in experience in that aspect for next role you will end up being offered lower title or role. That is when you hit the top bracket.
We call this the ol' bait and switch.
From an internal equity perspective, the top of the range is going to be for people who are moving to the next role or who are in the role for years, and bringing in someone with less experience at the same level could cause problems.
This is often a polite way of saying you're not good enough to get the top of the salary range.
The way you described what the recruiter said sounds more like negotiation to me (ie: you probably could get the max salary).
If they're softballing the explanation like that and qualifying it (they "prefer") there is still wiggle room.
If you try and they still don't bite that's kind of weird and mildly off-putting.

The range means you often enter near the bottom and earn your way up unless you have a lot of experience. If we hire near the top it can lead to internal equity issues.
