No! Sometimes it’s the hiring manager and sometimes the candidate. It’s hard to see how you’re at fault for events that take place once the candidate walks through the door.
No, there are many other checkpoints after the initial screen. However, it could reflect a broken interview process and it may fall to you to investigate (even if just to cover your own butt).

No. It doesn’t mean it’s your fault things didn’t work out because things can be great during the IV process, but you won’t know there’s a misalignment between the manager and person you placed until they’re in the environment.
Since there isn’t additional context, I hope that you’ve been checking in on this person and the manager since placing them in the role to see how things are going before it got to this point. If the manager isn’t happy earlier on the 3 month mark, it’s an opportunity
to work with the client to remedy the issues and save the hire, or start ramping up new candidates to get ready to backfill this person.
You can be a good recruiter with a good candidate and it still not work out. It’s sucks a lot, and I’ve definitely been there before.
The earlier you know, the better you can position yourself to avoid being caught off caught when the HM wants to pull the plug.
I don’t know if you’re in house recruiting or an external recruiter, but either way, routine touch points help in the long run.
