Why can't you transition at the agency you're currently at? Talking to your head of production would be the easiest transition path.
You'll need to start at a production assistant level there really is no other way.... unless you're very senior and you have close knit ties within the production dept who are willing to take you on as a junior producer... this is very rare and the chances are you will have to start at the bottom and work your way up...
Are you applying for mid-level producer roles or production coordinator roles? You'll probably need to start at the bottom as a production assistant.
Transition within your current agency. We've had account peeps do that here. Talk to your HOP and discuss. You will start out as an Associate Producer and work your way up.
If you have any ins with any production companies you could ask if they have any productions you can join them on. I've seen people ease their way in tagging along, playing PA and asking smart questions with out quitting their day jobs.
Networking within your current agency is your best bet. Will be tough to be hired into a department at a new agency without relationships since you're technically not really qualified for the job.
I've seen it a couple times. If you go to a smaller agency (wears many hats) or one that isn't very competitive, and make a good case for yourself (via nice website) you can probably manage to swing a jr or even mid level position depending on how much you know to start.
Just go to a consultancy. They have no titles.
