Definitely consult with several plaintiff side employees lawyers about their views of your case, and which forum to use for your claims.
I suspect your mental health issues arose from discriminatory behavior targeting you. So you likely have claims based on more than your mental health disabilities.
Your mental health issues could be considered your injuries due to discrimination (like your anxiety attacks) and thus tie into your damages analysis. Or their treatment exacerbated existing mental health issues.
If you haven’t already, please be sure to seek appropriate mental health and physical health care. Check out intensive outpatient programs and other high level of care programs. Documentation of your seeking and regularly engaging in care will also support your claims and damages valuations.
I am so very sorry that you have experienced this. So many have been deeply wounded by discrimination in the workplace. I hope you are able to find both helpful medical treatment and legal representation during this difficult time.

Yeah, just based on what you provided, I would move forward. Most times the pip are signed to show acknowledgement of receiving it, but I had numerous conversations about discrepancies, so I informed my firm that those conversations were my acknowledgement. So, that could be an argument for signing, opposed to saying you agreed, if needed. Can also included race and retaliation. No problem and good luck!
Did you have any accommodations, request accommodations, provided medical documentation to your employer related to your disability? Could be a case for disability discrimination, but could depend on the above etc. if bringing a charge of discrimination with the EEOC makes sense. I’m also assuming you didn’t sign a severance agreement. Also, if you signed performance write-ups, pips, etc may be a harder argument. For example, my firm pip’ed me and I simply refused to sign because I didn’t agree. I’m still employed for now, but I’m in 4 protected categories (race, gender, pregnancy, & accommodation) right now, so that should put them on notice that I will be filing a lawsuit by not signing.
I signed the PIP but I still got the accommodation with the medical documentation provided and I still was still let go by my firm. I requested the accommodation BEFORE the PIP began too. Thank you for your insight, my friend!
There is a strategy to filing EEOC complaints. A lawyer, not you, though you are a lawyer, shd do it. A pro per complaint, even & esp by lawyers in most (not all) cases shows weakness
