Just say you’ve accepted a position somewhere else that you’re very excited about, and you’re thankful for the opportunity to have worked with them.
Tell them your last day will be X/Y and you’re thankful for the opportunity to have worked with them.
Ask them to play 20 questions.
I’d keep it simple. Unless they grill you I wouldn’t mention anything about burnout. But really what’s the value in sharing anything more than what M1 posted above? You’re ready to move on. If they were letting you go the conversation would be short. No need to make it longer than it has to be.

It was super awkward. I still don’t know the best way to start it. But once it’s said it was good from there.
Did you already use your short term disability? I noticed you said you were leaving due to burnout and you were going to another role, try to use your STD benefit to at least buy you some more time at 100-60% pay.
If I go on leave then quit that seems bad? I’m planning to leave anyways
