I will usually take PTO to recharge but sometimes it still doesn't do the trick. If that doesn't work I will try to change my schedule a bit so I can allow myself some time to charge back up my batteries. There rae definitely times I am exhausted though I can tell you that much.

That's a great perspective! I believe it is healthy to get up and walk away for a breather, be it for 15 minutes, a few hours, or a few days. Running on empty doesn't help you or work product.
I got multiple jobs that fit together nicely and do not burn me to a crisp. I think that’s the only way to make it sustainable. I got burned out 3 times until I left consulting and took on two industry roles.
Also, AC1 why would you be ignoring the big companies? They’re the perfect settings in which you can become one of the worker bees. Startups tend to have far higher velocities of work and can accomplish in days the things that would take a bureaucracy months of coordination.
Technically speaking, you’re trying to maximize salary while minimizing responsibilities. So do you really care about being called a Sr vs a Jr if you get paid the same amount?
The funny thing I’ve noticed is that the higher up in a company you go, the less actual work you do. That’s the only reason I wouldn’t want to start off too low on the totem pole… ain’t nobody got time for the grunt work.
It could be some serious burnout. Yes, use your PTO indeed. One to get physical rest and to then decide if that is the job for you. You owe it to yourself.

I agree! No one else but yourself will take care of you until it may be too late. Take the PTO, smell the flowers, and eat the cake. We are never promised tomorrow, so live for today!
