They can't request your medical records, but they can ask for documentation from your provider confirming that you need remote work as an accommodation due to a disability. HR may give you a form (often called an ADA certification form) to give to your provider to fill out--you can Google this to see what the questions are--then you return the form to them. So long as the requested accommodation does not cause your an employer an undue hardship, they need to grant it.
One addendum to this - working from home has to actually accommodate whatever your alleged disability is. If whatever medical issue you’re dealing with isn’t abated or lessened by working from home instead of the office, then they don’t have to grant it.
