Any friends nearby? Bring in reinforcements to help plan a day or morning out. Have them take her for brunch, a walk, pedicure, etc.
If there is no one in the area to help do this covertly— would she be up for a spa day? Maybe a facial and a massage? Does she do yoga? Maybe taking a class would be a good step.
I second this. If she doesn’t have friends nearby, then plan the appointments and then simply let her know that you have planned some appointments that were already paid for.
If she’s not up for leaving the house, can you take your little one out so she can have some alone time at home? Maybe a park or playground, play date with a friend, visit to family, or something?

It’s a very good thing. She’s pregnant so maybe no tennis and drinking. But perhaps a weekly art class or something like that?
Hire a maid service to come clean the house for the day and use that as a reason to send her somewhere else and maybe go with her. Then she has the benefit of getting out and a clean house when she returns
