My agency does a “Good, Bad, Better, Best” format.
What went good?
What went bad?
What was better than the last time around, but still needs some work?
What was the best thing? (This can also be personal shout-outs.)
We schedule our “retrospectives” for an hour. First 15 minutes or so is everyone writing down post-it notes for each individual thing, then we go around and talk about them and categorize.
I like it because it starts out pretty positive, gets all the negative stuff out of the way, then ends on a high note.

Too often they become the focus of the thing while you’re in it. Some , for example, keep notes during the process to deflect blame and gather praise.
I can’t tell you to do that. I can just tell you that at least 1 other person will.
I call them retrospectives to remove the negative connotation. We discuss what went well and what could we do better. Similar to the method AS1 mentioned above.

Wait until the body is cold.
Stop calling them “Post Mortems.” The use of the term, in this way, is devoid of logic and lacks practicality. Words matter, ok?!
