If I'm referring people to an agency outside of my own, I fall somewhere between B/C. I try to help friends out by introducing them to other connections.
But for an agency I'm currently working at? The bar is a definitely higher. If they aren't a good fit, I'll hear about it early and often from colleagues.
I'll be honest though. My criteria changes based on the agency. You don't have to be amazing as long as I believe your talent matches the place you want to work for.
I've lost a friend/ex coworker for refusing to be a referral. He asked me to lie about why he left the agency we both worked at (he was fired for poor performance) which I said upfront I was uncomfortable doing. I don't feel bad about it.

C. How would a friend know if you didn't pass their info on vs. passed it on and just didn't get a reply from the HR person?
I'd focus on culture fit...and be careful of referring close personal friends (that can get political). And always declare your relationship!
The only time I won’t refer someone is if I truly think they wouldn’t fit and either they or the company would regret the move — and that extends to both acquaintances and friends.
Pro
If I truly think someone is great, I’ll go out of my way to help them. If I have no idea if they are great or not, I’ll still refer them so they can sink or swim on their own. Only if I actively dislike someone would I decline to refer them.
The idea that they reflect on me is ludicrous. As if anyone remembers how someone got the interview? And if they flame out and I’m still great, who cares?
It’s actually a sore spot with me, strangely, as I recently found out that a member of my family has NEVER given a reference or referral. His precious reputation prevents it, I guess. He’s pretty well known in his field, an academic who works at arguably the best program in the world and his assistants, students and mentees? Nope. They kiss his ass for years and do his grunt work and then they ask for a referral and he’s like, “nope, sorry.” Which once you have him on your resume, having him not refer you is almost like an anti-referral, as if you sucked.
I find myself mad on behalf of all those people! Dear peeps, I’m sorry on behalf of the whole family that you had to endure those holiday parties and whatnot and in the end looked a fool. His ego is just too fragile, I guess, to risk someone, somewhere not agreeing with his assessment of a candidate.

C! Unless the friend has shown skill in the thing they want then B but B with extreme caution. I’ve gotten really good at the polite decline to refer people I don’t know if anyone needs a line. Would also like to point out that referral bonuses often harm diversity in recruiting and are prob not the best way to evaluate a potential candidate.
