I had one candidate who didn't interview poorly per se, but I could tell she was really nervous. I felt like that might have been why it wasn't her best interview, so I still hired her. I could tell she would be a good fit for the team, and I was right.
The only things I can think of are: no better candidates. Strong referral. Urgency in hiring. But they all sound like bad choices.

The short answer, no I have not put a candidate through if its a bad interview. Sorry.
It depends on why they didn't interview well. If they seem to lack the fundamental knowledge required for the job then no, they aren't the right fit. If they didn't interview especially well because they were socially awkward, but still seemed to have the knowledge required, then I tend to not hold the awkwardness against them. Half our team is socially awkward, they'll fit right in.
