All of Old San Juan, and most especially the fortress, are amazing!
Condado District is great for nightlife.
Not a fan of Puerto Rican food so I don’t have any food recs, unfortunately.
I wouldn’t worry about not knowing Spanish- pretty much everyone in Puerto Rico speaks English. Very few exceptions.

OP, I have a couple of suggestions to add:
1. Plaza las Americas (the largest mall in the Caribbean).
2. Tibet Indigenous Museum and the Museum of Art of Ponce.
3. Caverns of Camuy and La Ventana Cave.
4. Pork Highway in Guavate. This is a road that is filled with local restaurants that specialize in pork and other island cuisine.
5. As other pointed out, the Bacardí tour is great. They show you the process of rum making, from cane sugar to bottle. You get some drink coupons, but don't have to drink if you don't.
6. For hikes, you have El Yunque. One hike that I did that was away from the normal path was El Toro, which is about 5 miles round trip, climbing to the top of a mountain. The view was amazing. I recommend getting the All Trails app. I found a bunch of hikes with that app. Also, the "Bosque Seco de Guanica" is another great hike.
7. Bioluminescence Bay. This one is great but it depends on the weather.
8. Coamo Thermal Hot Springs.
9. Any local festivals are a great way to experience local customs.
10. The Arecibo Observatory, great museum and science center nestled in the mountains.
11. Toro Verde Adventure Park - this is a zip line park. This was the one that Jimmy Fallon rode in PR.
All of these attractions are outside San Juan, but a short ride if you rent a car.
Also, you can get around the island without knowing Spanish. English is also taught at schools in the island, so you'll always find someone that will understand you.
This is really helpful. I am doing 2,3,4,5 for sure based on your suggestions. Is everything easy to reach by public transport or do I rent a car ?
