I don't have any advice but I'm sorry that has happened. Given it was unforeseen business reasons I don't think you should blame yourself too much.
Stay in touch with her. I’m sure she’d appreciate it if you help her navigate things at the agency, even after you leave. And who knows? You might end up working with her again one day!
Most importantly, it’s your happiness and you need to do what is best for yourself!
But, as someone who is currently on the receiving end of this, I encourage you to be honest and offer as much insight as you can to set her up for success before you leave. Let her know she can reach out for any questions, etc. even after you leave (if you’re comfortable with that). Shed as much light on any issues that contributed to your misery if she will be affected by them, and offer ways to approach them.

Yeah I think she'll be fine! It's nice that you care about her wellbeing but she's an adult who made her own decisions!

Well if you were so unhappy, why did you describe things as “great” to her? You basically lied to her.
No dramas. I suspect it's revenue pressure from the board that's funneling down combined with cracks that just start to show over time. Thing is, those cracks have been deal breakers for me and many others in the business - we're talking discrimination, bullying, etc.
