Your two weeks off won't refresh you at all? You may get MLK day off, and February you may get two days for the presidents' days. I'd try to save them for 4th quarter. We have no days off then.
Great for you, not so great for the kids. The only way I would do this is if I knew I had a really great sub who could handle it. Otherwise you might come back from vacation to a total mess.
I would do it if I felt I needed it... I have used FMLA to get a mental break. I feel that it was the best decision for me, my family, my co-workers and my students. Its better to plan to be off than to un-expectantly be off because of a breakdown.

PTO? Before making decisions read your contract carefully. We don't have PTO only sick days.
Plan it another time; kids are a lot near the holidays. It will end up screwing your coworkers more than the students.
Talk to your boss, take a week off at the end of Jan- beginning of Feb.
Tell them only that you have an out of town family responsibility. Leave exquisite sub plans with a prearranged sub.
Besides, traveling is cheaper further from the holidays while college kids and folks are all working.
Do you have to use them consecutively? I would plan 3 day weekends every week or every other week as needed. Also depending on how crazy the kids are that week.
2 yrs ago when I had a really rough year I took off every other Friday and I would plan to visit different cities/states. I left some practice from the lesson studied the day before or ACT math practice for the students (if fully completed extra credit on next quiz). They had to submit it online at the end of the day or by Monday morning. It was refreshing and I didn't come to craziness.
Why don’t you do 5 days? First day is getting back into the swing and then record videos of you teaching the lesson for your sub. Use the other 5 days for the 3 day weekends like some one said.
Doing so will not refresh you. You should use your PTO on counseling. It will help. Or find out what truly invigorates you about teaching and your life and make sure to incorporate those changes in your life.
You could even make yourself four LONG weekends throughout the year take a Friday off of 4 weeks spread out throughout the year and travel to a spa or do your garden.
You are mentally tired. More time off will not help that unless you have a clue as to what will refresh you.
Take mondays and fridays off I guess, but to peace out the beginning of the next semester, you are asking for a hellscape.
Maybe take like one or two extra days off but don’t take all the way till the 14th off. Honestly that’s just gonna make the kids and your coworkers eat you. Spread the PTO out, don’t take it all at once. Also your contract might for video to take the PTO all in a row so you might want to read that carefully. I would do as other suggested take like a three day weekend like every month or so. But I’m assuming you’re in public school and you get one week off in February and another week off in April. If you’re in private school you’ll get another two weeks off in March honestly if you’re taking this much time off I feel like you need to dig deep inside you as other people have said and ask yourself why you have to take this so much time off completely in a row. It’s not gonna do anybody very good. Parents are going to get upset colleagues are going to get upset kids are going to be upset…
It’s only a few days. If you’re ready to jump in when you return you’ll probably do better for the break.
