I swear I saw almost this exact same question there but it was someone they were in an actual relationship with. Either way if they're creating a hostile environment and you can't address it with them directly talk to HR

If you’re ready to leave, why not try to address the issue directly? Since they are your boss in a corporate setting, they might be uncomfortable with the reputational damage you are capable of enacting against them. Also makes for weird power dynamics if you are their direct report.
The goal is to communicate that it’s irrelevant whether you knew each other in a previous chapter of your life. You’re in the present and as your boss you want create mutual support which will help you work effectively in this role.

Only once, set up by a mutual friend at a party or something and even if you asked me what their name was then, I wouldn’t have remembered it. We both knew what it was. But as soon as I saw them in person it was a “oh shit” moment in my mind. See below comment for what’s making it hostile.

Definitely a weird vibe from them. Short and aggressive manner in conversations. Won’t answer any question directly, help strategize or pick up the phone when I call when I’m WFH but will promptly send a “what?” Slack message as the call gets missed. And same with the team.
Did you end it or did the other person end it? There’s always one person that fizzles it out. Or was it mutually agreed that neither wanted to hook up again?
That’s very awkward. Wish you knew who your boss or hiring manager was before you took the job. Can you transfer to a different position or department?
