Honesty - but always in the context of why these examples are at the detriment of the company. It makes it less personal.

I'd reiterate face to face and in email as formal notation
Be honest. Otherwise, everyone suffers. Just make sure you present your concerns as professionally and diplomatically as possible.
Honesty at your own risk. I was in a situation like this and execs specifically told me it was because they were two weeks away from firing the person and they needed more documentation. I gave it, they told the person what my complaints were, AND kept them on. You are the only person looking out for you.
I think you should be transparent but not disrespectful. You need to do your job and he won’t be able to hurt you if he himself has a bad performance review. Just do honest work!
