yes, Managing Director at PwC is a non-equity partner traditionally but recently it also serves as a stepping stone to partner, since here are so many directors these days.

No
MDs are near partners in role and are therefore a noticeable jump up from Director. They act as partners in some markets where a team might not have a partner, but cannot sign contracts and as P1 notes are non-equity and can later make partner.
Remember that roles vary by country in most firms. Some PwC European firms have a role called “Associate Director“ for instance that doesn’t at all exist in the US, nor do many have MDs. I believe I last encountered an Associate Partner in PwC Brazil but could be mistaken.
IBM also does an associate partner, but it's just a senior manager equivalent. I think BCG might have a similar title and/or multiple versions of "partner" but I forget how they interrelate.
Not at EY. After SM you have to go either exec director (non equity partner) or partner. there are aome rare cases of people jumping from exec director to partner.
