If you're well enough to start at your new firm, you're well enough to work? If not, finish out our leave at our current firm, and go back for like 2 weeks to deliver your notice.
It’s not uncommon for people to leave right after or before returning from parental leave. Check your leave policy to see if there’s a clawback provision.

I suggest not rushing your medical leave to start this new job, should you be offered it. It’s important that you he fully recovered so that you can excel at your new job.
Also, it may not be as simple to take leave once at the new job, because you typically need one year of employment to be eligible for FMLA leave. You can still apply for a leave as an ADA accommodation.
Whether your firm can claw back leave costs depends on its policies and the laws in your jurisdiction. You may want to return to work for 30 days before starting the new job: “An employee who returns to work for at least 30 calendar days is considered to have "returned" to work for the purposes of the FMLA.” https://webapps.dol.gov/elaws/whd/fmla/8d4.aspx
I know plenty who have done that. Just make sure no clawbacks or post leave requirement to stay or lose money
