I’m in Spain so process may differ but I made an appointment with a psychiatrist here and brought a note from my U.S. psychiatrist confirming my diagnosis. My psych here didn’t choose to re-test to diagnose and just prescribed me my meds.
Not in those countries but you generally need a GP to refer you to a psychiatrist. They assess and provide meds
UK you need an official diagnosis using a certified methodology to get prescribed, the process and wait times are long for NHS consults but you can always go private (though those are £££) But if you are already diagnosed and have your report and dosage sorted, you can transfer those reports to your GP who can then prescribe them for you without issue. We have concerta so that won’t be a problem to transfer across, but if you are on vyvanse, adderall or something US specific, then you’ll probably need to go to a psych to get the appropriate switch (and same considerations as above with long wait times)
I mean a formal ADHD diagnostic report completed by your psychiatrist which provides evidence of your diagnosis against established criteria e.g. dsm-5, diva etc. There are a couple that are recognized in the UK, so you will need that to transfer across to the NHS. It’s essentially examples from your childhood / as an adult of how adhd disrupts your life. We love medical paperwork here, but honestly it really boils down to your GP too, so choose wisely

Keep in mind that not all medications are available in all countries. I lived in Belgium and there was only a limited range of meds that practitioners were allowed to prescribe/were covered by insurance.
