I would talk to SM or Partner (whoever you feel comfortable with) and say I am trying to take care of my mental health and need this time one day a week. No one should say no. You shouldn’t have to say anything more
You don’t need a note, just communicate your schedule up front, share how you plan to complete the work (like you said by coming in earlier or working later afterwards as needed) and do what you say you will do.
Do not in any way give up going to therapy. You have every right to leave for an hour and a half one time a week no matter what season it is. And no you don’t need to tell him what you’re going for. You could easily say weekly doctors appointment or tell someone else other than him on your team.

OP, my senior on my current engagement is going through something similar. She spoke to me (her manager) and our SM. She's a hard worker so we aren't worried about her work getting done but we definitely are concerned and care about her mental wellbeing and she being OK and "whole" . Neither of us was even concerned about her sessions twice a week and neither of us needed to know what it was about. You have nothing to worry about and nothing to declare. Just communicate it to your SM clearly.

Email your Manager telling him you have a weekly medical appointment and that you need to leave by 6:30 pm on those days. Say 6:30 incase you get delayed getting out of the office, you don't want to cut it too close and be frazzled/rushing into therapy. You don't have to tell him it's therapy, just say a medical appointment. Take the time to take care of yourself. CC your SM, PPED, and People Consultant on the email. You can go into more detail with the People Consultant about the nature of the ongoing appointments, but you're under NO obligation to share details with your teams.
...that I would like to leave at 7pm once a week to go to therapy? Is it possible to not have to tell him what I go for? He is pretty judgmental and I from what I can tell, not super supportive of sexual assault survivors (just my interpretation from him talking about not believing Christine Ford). I feel bad leaving the team early once a week, but I am willing to come in early on other days or work extra on the weekend to make up the hours. I just don’t want to lose the progress I have made these past few months and want the flashbacks that I have been experiencing to stop
You can get a doctors note from the therapist without the therapist giving any detail to what it is you need to attend to. If they want to do their research on the doctor who signed-off on the note, that’s on them but it is just as valid as any physical illness and with a doctor’s note you have the documentation you need to back it up.
I would just say you have an important commitment that can’t be moved. Tell them you’ll come in early the next day or stay after the rest of week.(you likely won’t have to). I would say where you are going. Too many people share other people’s business. You got enough on your plate. From a lifer.
I can’t believe you have to work until 11 pm in the first place. How is that productive? Sorry you have to do that

I’m not sure the situation or group you are in but I have never in my career has set hours for staff that went until 11 pm! I’m not saying team and I have never worked that late, but having expectations of working until 11 pm every night is totally counterproductive. I’d consider looking for something more reasonable. It’s out there!
