Please confirm with a medical professional if mixing AZ and Pfizer is okay. Not sure if they have similar vaccine cards in India, but I’d just schedule a new appointment for them, i.e. like they’re getting the vaccine for the first time. This way they get the CDC vaccine card and have no issues getting a booster. Try Giant or Costco. Got mine at Giant.
D1 the amount is not different for adults, just children
typically no one is asking for any real info other than proof of id. you should be able to schedule normally
I second D2 & M1: we recently tried this, went to multiple pharmacies and all advices against taking the booster shots. My parents took Covishield in India as well
Lots of studies now that booster mix and match is totally fine: https://www.nytimes.com/2021/12/02/health/covid-booster-shots-mix-and-match.amp.html?

Have you looked at the CDC site? Do they have guidance? Or the Astrazeneca site?
You could also call any university health center and ask.
CDC has “no guidance” on this apparently. I spoke to a pharmacist and Walmart and they’ve been told that if you have received any vaccine other than the 3 mentioned above then you don’t need a booster.
I’d probably just schedule it as a first dose of Pfizer and call it a day. It’s the same dosage. (Which isn’t true for Moderna.)
This is what my family did. Just say you’re looking for a first dose (they had AZ and then Pfizer boosters)
