I opened a Vanguard account 5 years ago and dump everything into their total stock index fund (VTSAX). You can’t go wrong with Fidelity and Schwab either. All you’re looking for is something with a cheap expense ratio. Tons of resources comparing all the options and what’s best for your situation. Pro tip- have payroll automatically send a portion of your pay to your account and get to living without it. Ideally max it out since it’s only $6k/yr.
A1 above explains it well.
Instead of trying to "split" 401(k) and the Roth IRA, think of the Roth as a separate, post tax bucket to save/invest. You can open an account in Fidelity, Schwab or Vanguard.
Try to max out 401(k) first - $19k in 2019. Then $6k in Roth.
Open a Roth IRA with fidelity...and start contributing to it and pick investments? I don’t understand your question
Post tax, and income restricted.

Likely no difference between the two.. special circumstances do exist, like taking a loan out from a 401k to buy a house, etc. Also if you expect your tax rate to be higher in retirement than it is now, Roth is marginally superior.
But most likely if you are not maxing out your 401k, you will get little/no lift from splitting your contribution between 401k and an. IRA.
