I had melasma above my eyebrows right in the middle and it made me look like I had a unibrow LOL it finally went away after a year of consistent, no more like religious, tret every night and sunscreen (&reapply) and hats/sun protection. I feel your pain. The sun protection I think is what made the biggest difference
Treatment Strategies
1. Sun Protection
• Broad-spectrum sunscreen (SPF 30+ mineral formulas; zinc oxide/titanium dioxide).
• Physical barriers: wide-brimmed hats, UPF fabrics.
2. Topical Agents
• Hydroquinone 2–4% (gold standard) often combined with retinoids and mild steroids in a “triple combination.”
• Azelaic acid 15–20% or kojic acid for patients who can’t tolerate hydroquinone.
• Niacinamide 4% serums to reduce melanosome transfer.
3. Procedural Options (only once stable with topicals)
• Low-energy chemical peels (glycolic or lactic acid).
• Gentle Q-switched or picosecond laser/IPL (in select skin types).
4. Maintenance
• Continue broad-spectrum sunscreen daily.
• Rotate or taper topical depigmenting agents to minimize irritation and rebound.
6. Tips for Better Results
• Patience & consistency: Visible improvement often takes 8–12 weeks.
• Avoid irritants: Harsh scrubs, fragranced products, and steroid creams around the mouth.
• Layering products: Apply sunscreen as the last step, 15–20 minutes after any active topicals.

Bottom line: Mustache melasma is simply melasma in the philtrum region. With diligent sun protection, a targeted topical regimen, and—if needed—gentle procedural support, most people see a marked lightening over 2–4 months.
