Mostly lay offs due to revenue shortfalls or budget cuts (not truly a firing but feels the same way to the person affected). But for cause, I have seen: an affair with an underling that became public knowledge ( and rumors that he was diddling more than that woman), stealing from petty cash/expense funds, sharing racist emails, just being bad at their job, and one person for having a huge file associated with them linking them to all sorts of poor behavior from gender/weight/race discrimation to causing a woman to try to kill herself.
Nope just genuinely interested in learning from the faults of others.
Saw a CD get fired because our POS, incompetent GAD threw the CD under the bus completely undeservedly.

I feel like getting actually fired doesn't happen too often. The only one I've witnessed was a creative who snuck something inappropriate (I never knew what) into some work that almost* made it in front of the client. Must have been super inappropriate...
Mostly seen just layoffs, but the two times I know of people being fired, it was for dropping the ball while being out sick and not getting coverage for a shoot, and the other was for sexual harassment
In short, people get fired for the usual, expected reasons, ad agencies are no different from anywhere else. Even the CD who was fired by incompetent super - pretty much standard practice. (Former GSW.)

No, they all went through the probation process and were escorted away from the office when we weren't there.
I've seen a few people get out-right fired - general incompetency (seen this a few times), not high enough billing %, boss hated them. Had one coworker fired for general incompetency and it was warranted, to the point that some people questioned whether she lied on her resume.
Fired 2 of my reports after discovering their flame wars on slack. Slack has taken out a number of staffers over the past year.
