I would state what you said to him in writing. It’s extremely rude to take calls when you have scheduled something on your calendar in advance. It’s especially rude to be late. If he is a good boss, he will realize his error after you bring it to his attention and do his best to change his ways.
I let my boss, delegates, and coworkers know that I am also busy with things to do for work, so if they are more than 5 minutes late, we will have to reschedule the meeting. Oftentimes folks forget that we are human and have emotions. The things they do can make us feel undervalued and under appreciated. But the squeaky wheel gets the oil. Speak up!
I would just talk to him. Let him know that your time is valuable and if you’re going to have one on one’s, then you would appreciate his undivided attention while you cover what you need to. I always find that it’s better to just come out and say what you need to. It most likely hasn’t occurred to him at all.
I would speak to him directly. Lead with compassion and empathy. Let him know you understand how busy he is and that he has a lot on his plate to juggle. Mention that in order to fully support him to the best of your ability, it's important to you that you have his undivided attention during your 1:1 time together. Sometimes, the job duty as an EA/AA is to literally hold their hands and redirect their attention.
It's totally normal and valid that you feel unappreciated and not a priority. My sense is that he/she is very appreciate of you, your support, and what you do for him/her. It's likely they've just lost touch and just need a little help to see your shining bright light again. :)

your boss sounds stressed out and afraid to not take the calls of his boss. Maybe you could offer to help with time management for your boss and ask him is there a better time for him to do one on one meeting with you maybe virtual?
