Use a different recruiting firm
You said you gave one of their clients a bad review because they treated their employees like garbage. Was this on social media? If it was, prospective employers might be checking your social media posts and don’t like what they see. How did you word your review? It could make companies decide they want nothing to do with you.

Unless you are from a really small city it shouldn't matter if the biggest firm doesn't want to work with you. I personally choose not to work with the biggest firm because I don't like all the noise in the background when they call me. At least in Boston, there are dozens of accounting recruiting firms.
It just doesn't make sense that this company is sabotaging you 3 years later. The only way it makes sense that a firm could black ball you is if you work in Topeka or an equally small city where everyone knows each other.
What are you saying in the interview when they ask why you left your previous employer or why you have such a long gap in your resume?
People, don’t waste your time and give this clown any more advise. Look at how he has responded to some of the comments. It quite obvious he is the problem. This is why he’s having issues with recruiters and finding a job.
this group of people telling you that it’s all in your mind, is why you dont ask questions like that on social media. A bunch of snarky dullards who are indicative of why the pipeline issue exists.
If in fact you are being blackballed (it is possible), keep networking, do contract work and keep working in smaller environment. The worthwhile employers know that nasty things happen. If your work & attitude are good, you’ll break through eventually. Never speak about it either. Leave it in the past.

OP you are clearly unhinged and in need of counseling. No wonder no one is hiring you.
Any way you can network at CPA events - maybe get involved and you will meet someone who could help you. Can you contact your college alumni office? Any friends that are accountants and can recommend you to their firm?
Are you able to say why you think you’re being blackballed? Perhaps we can then work back from there to figure out how you may manage it.
At what point do things seem to be falling apart?
Also, is relocating an option? Perhaps applying for roles in other cities and then only moving when you’ve got something?
If you didn’t work too long with them, and you’ve been off work for a bit, anyway, can you explore omitting that last employee in your next few applications and seeing if that makes any difference?
