Help - how do I fight claims by scammer healthcare providers?

I recently went to 2 different providers.

Dentist, made appt via Zocdoc to see a dentist that was in network. Another dentist at the location saw me instead who was out of network. I have text evidence they said it would be covered before visit. Claim was denied - asking for ~500 dollars.

Saw a specialist who’s office staff verified my insurance would fully cover the claim. Get the claim, says they are out of network. Owe 600.

Had a glowing fantastic performance review last year. This year no review, just a meeting with managing partner and HR to tell me I am fired and to leave now. No bad performance. Is it just the money?

I was promised a promotion, then was told I wasn’t getting promoted, started applying to other places, fought for it and ended up getting promoted and a raise. I was just offered a job at another agency for $15k higher than the raise I just got. Throughout my career, I have worked at both “traditional” and “digital” shops. My current agency would fall under traditional but also offers digital services. Our process, roles and skillsets seem way outdated but I wasn’t sure if it was just my opinion or if this is just how it works. For example, we have no CTO or tech leads. Our digital producers feel more like project managers than actual producers. Account Management is just traditional, no real digital knowledge. Feels layered and dated.
