Own your balance. Tell them you have an appointment and go to your car. Prioritize yourself.
You can do it during a lunch break window and grab lunch on the way back to eat at your desk or you can just let your manager know you have PT if you need regularly recurring sessions and lunch hour doesn't work
When I legitimately had to leave work for PT once a week my PT guy only had available during office hours 🤷♂️ your fake excuse could be my real experience haha

Put it on your calendar as a recurring appointment that shows you as OOO. If you want, tell your manager you have a standing weekly medical appointment at that time. What you’re doing isn’t anyone’s business.
This, + try to get a standing appointment that is at the beginning or end of the day, so you can condition people to know that “on tuesdays (eg), Bob doesn’t come in until 9:30/leaves at 4.”
You can take a session on weekends. I know a therapist who specifically books sessions on weekends for working professionals. If you need details, let me know.
I would do it in the car, if you're thinking people may listen then drive somewhere with a lot and park in no mam's land.
If you don't have a car, you may be able to ask HR for assistance.
Can you ask to work from home one day? I know I’m sometimes a wreck after a session so can’t imagine having to go into office after that

Not therapy related, but when I had to pump and the client didn’t have a mother’s room (or it was occupied) I’d do it in my car.
