Agree with BCG, dress and a blazer. If you feel too formal you can take the blazer off during lunch, but I think it's better to be too formal than too casual
A dress and blazer or cardigan would work well!
I always feel dress with blazer is a good way to dress up without looking like a "suit"
Is that lunch during a work day or on the weekend? If it's during a work day then go to it in your normal business clothes that you would be wearing that day

I would say nice pants and top but no blazer/jacket. Of course others will disagree with me, but I think you should wear whatever you are most comfortable in. Besides it's like 100 degrees!
Is there something in between for females? I'm not too familiar with formal attire. For example, if this was me, my workday outfit is a shirt and dress pants. An interview outfit is a full suit and tie. A pizzeria lunch interview, I would go somewhere in the middle and wear a suit jacket with my shirt and pants

If the recruiter suggested a suit that's what I would wear. But make sure the shirt you wear under the jacket is appropriate to wear on its own in case you end up wanting to take the jacket off. Again I usually err on the side of being too formal because I feel it's better than being too casual.
