Vacation policy is judge specific. A lot of judges frown upon any sort of vacation since you’re only there for a very limited time
Federal health insurance is great. I had BCBS (not in NY) but you get a lot of options as a federal employee. My judge didn’t allow vacations.

I had GEHA and never had issues with it. My vacation was judge specific too. I was there for 2 years so had a little more vacation than others

As for vacation, I would not plan a vacation that’s more involved than a long weekend unless there’s a specific time frame chambers will be closed and you clear it ahead of time. I did 2 clerkships, circuit and district. Circuit judge took a week off during the holidays so all the clerks did too. During my district court clerkship it would have been a really big problem if the clerks were not all in chambers unless traveling for a wedding or something. I worked while traveling home for Christmas but would not consider that a vacation. I think I would have been fired if I tried to go somewhere for a week.
Edit to clarify, term clerks are known as exempt employees for federal leave, so they do not accrue paid leave time like full time court employees. Meaning any time off is totally at the discretion of your judge.
My two year term started at the end of 2021 and I was the first term clerk that was under the Federal Leave Act for my Judge. I accrue half a day off for every two weeks I work
My judge doesn't allow for any vacation--I got 2 days for Xmas because I scheduled with co-clerks, and I got a day off for a funeral that was out of the country. Most do, however, particularly if you are planning much ahead of time. Talk with the clerks there. You are exempt from the LEAVE Act which means that your judge can do whatever they want.
I also had GEHA, its annoying because it won't work at Walgreens, but other than that it works well. I made a spreadsheet of my possible co-pays and doctors appointments to decide which one was best. Depending on region, GEHA or BlueCross give best coverage for price, but if you want cheaper there are many options.
