Might need more details about how long you've been there etc. I'd say leave. I'm always sketchy about counter offers. Feels like you've got a target on your head when the next round of layoffs comes. Change is almost always good. Go for it!
Never accept a counter offer. There's a reason you decided to leave. Will the counter fix all that? Nowadays you can easily jump from job to job to achieve happiness, not like before where it was frowned upon.

'Very unhappy' - leave - a counter offer is nice and makes you feel valued, but it's a sticking plaster - it won't change the root of the problem
Leave. I find that counters tend to resemble future punishments.
A counter offer means they could have paid you more all along and didn't. Never accept a counter, especially if you're unhappy. The money won't change anything
Counter offers is like telling a significant other you cheated because you're not in love anymore and they say I forgive you please don't go I'll change. You'll always be wondering if they're plotting against you, especially if the number seems substantial and something extremely unexpected. Don't look back. Something drove you to start looking in the first place. Take the money out of your decision and make it about your mental health and happiness.

Leave. If you've raised these issues previously and nothing changed, which made look elsewhere, then take the jump and enjoy a new challenge
Leave and don't look back. If new place and new job not good either, look for another again... Move forward

I did try using my counter to get the new employer to up their offer. It didn't work but I was still going to make more than I was at my current position and I read an article on Forbes on why you should never accept a counter so I said fuck it and left anyway.
