I told my manager I have a standing weekly dr appt and have to leave at 5 on wed. I come in an hour early in wes and I block out that time each week on my calendar so no one books late mtgs. And I refuse to feel weird or guilty about it. I just make sure my work is done or handed off or I can pick it back up from home after my appt. I also shut down coworkers that ask with a very deadpan I have therapy. Most are like oh and feel awkward so leave it alone.
Block the time out in your calendar (recurring) and send an email to all people that may be affected by you leaving early. Feel free to even add them to the event in your calendar so they have it in theirs. You don’t have to tell them you’re going to therapy. But I would usually tell them it’s a standing appointment for physical therapy - for my back or whatever. That way, no one really pries.
I set mine up on the weekend. Otherwise, I have a relationship with my CD who knows that sometimes I have to leave the office and be MIA for an hour a week.
My therapist has later hours till 6:30-7 so perhaps that may be helpful for you?

Seconding the Weekly Doctor Appt approach. I told my bosses that I can reschedule it if a mandatory meeting comes up, by one year in and it hasn’t been an issue ever.
Let key people know. Block your calendar. Like @Prod1 said, it doesn't hurt to answer questions. I tell folks it's why I'm my happy self with a smile, and I usually get folks asking for a therapist recommend.
