Mental health is actively being recognized as a real issue so bringing it up shouldn't be a big issue.

How big is your employer? Check out your HR Policy and details about requesting reasonable accommodation.
As VP1 mentioned, if you are in a bigger organization, it should be easy to reach out for accommodation. If it's a smaller org, you might want to see if you can get a feel for how people might respond. They should be okay with it, but should doesn't always mean that they will.
File for FMLA for yourself for mental wellness
OP - FYI, FMLA only applies for employers with 50+ employees within a 75mile radius. And it’s unpaid leave.
Again, look into your HR policy. Reasonable accommodation will help if you want to continue working but need some changes to how/when you work. Granting it is up to the employer but if you have a decent one and your accommodation request is not outrageous, it should be fine.

You're mental health is completely relevant and worth taking the time to talk about if it's affecting your performance. People aren't robots. We have needs and requirements to function.
It's the same as any other health issue. Not every employer is going to be understanding, but if it is noticeably affecting your work I might bring it up.
