If it's a prospective employer, I wouldn't disclose simply because they are not meant to discriminate but most of them do. As for your employer, disclosure is supposed to come with some accommodations and protection from discrimination, so it should be in your best interests to share the information. You have to do what feels right for you.

Wait until necessary. I would also disclose it in writing when you eventually do.
Agree that it is kind of a double edged sword. You run the risk of being discriminated against, and proving that can be incredibly difficult. However not saying anything can lead to issues once you get in there and do need to now get it documented. I can appreciate both sides and neither one is easy, really depends on how you are working with.
