EY2-even if you're a male, gynecologist. In today's world we refer to gender as however we are feeling on a particular day. But in all honesty...sick day. Woke up. Not feeling well. No advance notice necessary
I would say the job search might take longer than you anticipate. 4-6 weeks would be a quick turnaround for applying and receiving an offer. I would limit the number of times you say you have a doctors appointment.
I normally went to a meeting room and blocked my calendar for two hours so no one finds me. If they ask, I say Im on a call. If going into office interview, I'll just take a vacation day.
If you use appointment excuses, remember there's more than the doctor's to say. Dentist. Eye doctor. If you're female, gynocologist. Try to schedule them early in the morning and you could say you're sick. When I was changing, it wasn't possible for me to use a day of vacation so I got my interviews scheduled either early morning or late in the afternoon to just help accommodate a little better.
I did my interviews while available. Also, always make it seem like you have too much on your plate so that you have more available time. I was on two clients and I would just say that I would work from the office or at the other client (unless they absolutely needed me there in person). The process took me around 4 weeks to receive my offer. Good luck!!!
Also, less is more. Don't give elaborate stories unless people ask for details because it becomes increasingly obvious.
Just take a sick day. It's flu season now. Nobody wants sick people around.
Try to schedule your interview early in the morning. Then just make an excuse for arriving late. It's easier than making an excuse for not coming at all imo

Or say you are working remotely because you have some sort of appointment.
