It depends on your plan, but it’s usually the same rate of co-pay as a specialist(ie dermatologist). Check if you have and in network or out of network plan. If you don’t have any out of network coverage you need to call your insurance company and have them send you a list of therapists in your area that take your insurance.

I've read we get 12 free sessions per year if they're in network. And we get unlimited (until December) for virtual appointments.
Head to our bowl and search "therapy" and you'll find a lot of info.

Would an HSA cover it? As in will the withdrawals be tax and penalty free if used for therapy? Google is being unclear and is making it seem like if my primary care recommended me to therapy then the HSA would cover it.
HSA will cover it but you will want insurance to cover it's part too and go towards you out of pocket max.
Usually EAP (Employee Assistance Program) is separate from medical insurance and they refer you to therapists and cover between 8-12 in person sessions or 20 virtual sessions from BetterHelp.
To add, BetterHelp is not covered by insurance but the sessions are $60-90 for 45 minutes. Can use HSA for it.

FYI, if you discuss mental health concerns with your primary care provider, your insurance will probably count that as a mental health appointment; so if they only allow 10 visits for year, you would only have 9 left. Good question! Each plan is different so you should check your current Certificate of Coverage. Benefits change from one year to the next so it is important that you find the one that's most up to date.
Completely dependant on the plan. Some plans have deductible and coinsurance for everything, some plans have copays. Copays can vary widely by plan as well. Best to contact your insurance provider and ask them for mental health benefits info.
I think all major health insurance providers have some form of coverage for therapy, as mental health is considered a medical concern. The only way to tell is to call your insurance company to get verification. They probably have some type of coverage though
