Honestly, there comes a point where you’ve done all you can. If the family refuses essential treatment despite understanding the risks, maybe it’s time to consider involving child protective services. It’s harsh, but refusing necessary medical care can be seen as neglect. You have a responsibility to advocate for your patient’s health, even if it means taking uncomfortable steps.

I’ve been in similar situations, and it’s definitely challenging. Sometimes bringing in a patient advocate or a counselor can help. They might connect with the family on a different level and address their fears more effectively. Also, providing written materials or even connecting them with other families who’ve gone through the surgery can make a big difference. Keep showing empathy—sometimes patience and understanding are the best tools we have.

Just accepted an offer with a psych startup. Not exactly the offer I was hoping for as I’m taking a slight pay cut ($73/hr) and it’s a 1099 position. I keep getting stuck on the 1099 piece because I’ll be working 40 hrs/wk, but have been told I’m not eligible to be a W2 employee. Needless to say, no benefits whatsoever. I'm a PA in the PNW working for a private group staffing ED's in several hospitals. Recently the group has asked the PA's to take on a couple new roles (triaging and rapid track from triage i.e. straightforward ESI 4, 5). With these changes the PA's are considering asking for increased pay in the form of an addition of a COLA (cost of living adjustment) to our normal salary. Was wondering if any other PA's in the ED have these in their contracts and at what rate.
