I would request the reviewer to provide a recap of the discussion and also ask them to provide you with specific details about what has gone wrong and ideas for how you can make it better.
You should be offered ample opportunity to improve, so do the work and document what you’re doing, to address the concerns. Ask for a reconnect check in to see how things are going in X amount of time to get feedback, and document that as well.
Bottom line: documentation is key. If it isn’t documented it didn’t happen, or at least it’s one person’s word against the other’s. It’s always a good rule of thumb to recap this type of thing in writing and make sure it sent through company email so there is a record trail.
