Just tell him what you’ve posted. He may reply that he wants to retire and may do a deal with you to take over his practice at a reasonable price. It’s often best to be straight to the point and tell it as it is.
If he’s forgetful then the practice is at risk of a PI claim. You need to raise this for your own protection too. Even if he is emotional, you must do so. I think it more likely after an initial reaction he will agree with you and discuss exit terms. If he doesn’t then you know you must leave. But deep down he’ll know the truth and is more likely to welcome the issue being addressed.
Quit. Go work somewhere else, or expand your business. Make tons of money and have a great life.
Paying someone to do a job, even for 20+ years, doesn't earn you their undying loyalty when you disengage with your business.
He already thinks you're not loyal. Leave.
Remember Maya Angelou’s advice, “When someone shows you who they are, believe them the first time.”
Take her advice and move on.
