I had something similar happen after giving birth/very delayed insurance submissions (on the hospital end). I just called hospital and reminded them it should have gone to the other company, they resubmitted, no issue
Yes they can cover retroactive, call your hospital billing department and have them resubmit/explain their error and that you had a different provider at the time (and for a number of months after)
Your previous coverage should pay it, you might have to jump through quite a few hoops to get them to, but don’t give up.
